- Must be a Filipino citizen and a certified resident of San Juan City
- Between 18 to 59 years old
- High School Graduate or ALS (Alternative Learning System) Graduate

- 4 pieces Passport-size photos (White background, formal polo/blouse with digital nametag)
- 3 pieces 1x1 photos (White background)
- 2 pieces Photocopy of Valid Government IDs
- 1 copy Photocopy of PSA Birth Certificate
- 1 copy Photocopy of PSA Marriage Contract (for married female enrollees)
