How to apply Unified Multi-Purpose ID (UMID card)

Are you familiar with the UMID card or the Unified Multi-Purpose ID Card?
It’s very useful ID, in case you needed one to present in any business transaction.
This UMID card will serve as a single ID of all SSS, GSIS, PHILHEALTH and PAG-IBIG members.
The government employees can apply in the GSIS Enrollment Center, while private employees and OFWs can apply by going to the nearest SSS branch near them.
Why UMID card is essential?
  • It is considered as a valid ID as Primary ID and an acceptable ID.
  • To transact easily in government offices and agencies –SSS, GSIS, Pagibig and Philhealth.
  • One tap on it at the GSIS Gwaps/Kiosk, you can apply for a loan, cash advance or view your account, you can use it on your SSS transactions, Pagibig and Philhealth too.
  • Another benefit to the card is that you can view your SSS data at any SSS kiosk, including your employee static data, loan status and balance, premiums and history.
To know the steps in getting UMID card, let me give you an instruction on how the process go. It’s easier to follow, so you can grab your own UMID card.
Step 1: Accomplish first and fill up the application form.
Step 2: Bring at least one (1) of the following primary IDs.

  • Driver’s License
  • Passport
  • Professional Regulation Commission Card (PRC Card)
  • Seaman’s Book or Seafarer’s Identification and Record Book
In absence of any of the above IDs, kindly present at least two (2) of any of the documents listed:
  • Company ID
  • School ID
  • Postal ID
  • PhilHealth ID Card
  • GSIS Card
  • GSIS Certificate of Membership
  • Tax Identification Card
  • Voter’s Identification Card
  • NSO Birth Certificate
  • Marriage Contract
  • NBI Clearance
  • Police Clearance
  • Senior Citizen Card
  • Pag-IBIG Member’s Data Form

  • Overseas Workers Welfare Administration (OWWA) Card
  • ID Card issued by Local Government Units (e.g. Barangay, Municipality, or City)
  • ID Card issued by Professional Associations
  • Baptismal Certificate
  • Alien Certificate of Registration
  • Transcript of School Records
  • Permit to Carry Firearms issued by the Firearms & Explosive Unit of PNP
  • Temporary License issued by Land Transportation Office (LTO)
  • Permanent Residency ID
  • Certificate from:
  • Office of the Southern/Northern Cultural Communities
  • Office of Muslim Affairs
  • Certificate of Licensure/Qualification Documents/Seafarer’s ID & Record Book from Maritime Industry Authority
  • Certificate of Naturalization from the Bureau of Immigration
  • Bank Account Passbook
Step 3: Visit any of the following Philippine government agencies
  • Social Security System (SSS) – See all SSS Offices within Metro Manila
  • PhilHealth
  • Government Service Insurance System (GSIS)
  • Pag-Ibig – See all Pag-Ibig Offices within Metro Manila
Submit the requirements needed, the application forms and photocopies of IDs. Just wait until your papers get verified.
After your verification, you can now proceed to the photo booth for the picture of your ID, then have your signature and fingerprint. Then, wait your UMID ID for approximately 30 days via mail.
Step 4: You need to activate the card or else it will be useless. Your card will be activated 72 hours from the time you validate your fingerprints at the GSIS Gw@ps Kiosk. You can always ask staff there on how to activate unless you see yourself wondering how that is.
