Taguig City to hold Passport On Wheels

The Department of Foreign Affairs (DFA), in partnership with the City Government of Taguig, is opening 1,500 slots for those who wish to avail of the upcoming Passport on Wheels this October 21, 2019, Monday, at the Taguig Lakeshore Hall, Barangay Lower Bicutan in Taguig City.
The services can be availed of from 8:30 a.m. to 4:30 p.m.

Taguig resident-applicants must personally present their requirements (See list below) to the Civil Registry Office during the application period from September 9 to September 20, 2019 (except Saturdays, Sundays, and holidays).

Please be reminded that we can only accommodate 200 applicants per day.

REQUIREMENTS FOR REGULAR PASSPORT APPLICATION

ADULT NEW APPLICANT


  • Original copy of both Philippine Statistics Authority (PSA) Authenticated Birth Certificate on Security Paper (Local Copy from Local Civil Registry (LCR) copy is a MUST for those with blurred/unreadable PSA birth certificates).
  • Married Females must also submit Original copy of PSA Authenticated Marriage Contract on Security Paper (Local Copy from Local Civil Registry (LCR) is a MUST for those with blurred/unreadable PSA marriage certificates).
  • Original and photocopy of one (1) acceptable Valid ID

ADULT RENEWAL APPLICANT

A. ePassports – Passport numbers starting with ‘E’

  • Original and Photocopy of ePassport
  • If there is a change of status, married females must also submit Original copy of PSA Authenticated Marriage Contract on Security Paper.
  • If there is any correction/annotation, applicants must also bring both of their Original copies of PSA Authenticated Marriage Contract and PSA Authenticated Birth Certificate on Security Paper.


B. Older Passports – Declared as Renewal But Always Treated as New

  • Original copy of Philippine Statistics Authority (PSA) Authenticated Birth Certificate on Security Paper. Please note that a Local Copy from Local Civil Registry (LCR) is a MUST for those with blurred/unreadable PSA birth certificates.
  • Married Females must also submit Original copy of PSA Authenticated Marriage Contract on Security Paper. Please note that a Local Copy from Local Civil Registry (LCR) is a MUST for those with blurred/unreadable PSA marriage certificates.
  • Original and photocopy of one (1) acceptable Valid ID.
  • Bring the original and photocopy of your old passport.


MINOR NEW APPLICANT


  • Original copy of Philippine Statistics Authority (PSA) Authenticated Birth Certificate on Security Paper. Please note that a Local Copy from Local Civil Registry (LCR) is a MUST for those with blurred/unreadable PSA birth certificates.
  • Original copy of PSA Authenticated Marriage Contract on Security Paper. Please note that a Local Copy from Local Civil Registry (LCR) is a MUST for those with blurred/unreadable PSA marriage certificates.
  • Original and photocopy of one (1) acceptable Valid ID of either parent
  • Minors aged 8 and above have to submit an original and photocopy of their school ID (If not available, present Form 137 from school with the recent photo of child and dry seal. Please note that dry seal should hit the photo of the child.)


NOTE: Applicants are advised that minor applicants must be accompanied by the Mother to avoid supplemental documents (i.e. PSA Marriage Certificate, Special Power of Attorney, Affidavit of Support and Consent, DSWD Clearance, etc)

RENEWAL MINOR APPLICANT

NOTE: Always treated as new applicant; regardless of the type of passport.


  • Original copy of Philippine Statistics Authority (PSA) Authenticated Birth Certificate on Security Paper. Please note that a Local Copy from Local Civil Registry (LCR) is a MUST for those with blurred/unreadable PSA birth certificates.
  • Original copy of PSA Authenticated Marriage Contract on Security Paper. Please note that a Local Copy from Local Civil Registry (LCR) is a MUST for those with blurred/unreadable PSA marriage certificates.
  • Original and photocopy of one (1) acceptable Valid ID of either parent

PASSPORT FEES


  • Passport Fee: ₱1,200 (will be paid through e-payment)
  • Courier Fee: ₱150 (will be paid on October 21)
  • Convenience Fee: ₱50 (will be paid through e-payment)

  • Social Security System (SSS) / Government Service Insurance System (GSIS) Unified Multi-Purpose Identification (UMID) Card
  • Postal ID
  • Land Transportation Office (LTO) Driver’s License
  • Student Permit (Card Format)
  • Professional Regulatory Commission (PRC) ID
  • Overseas Workers Welfare Administration (OWWA) / Integrated Department of Labor and Employment (iDOLE) card
  • Commission on Elections (COMELEC) Voter’s ID
  • Voter’s Certificate (with voting record, dry seal and signed by an Election Officer)
  • Philippine National Police (PNP) Firearms License
  • Senior Citizen ID
  • School ID (enrolled or fresh graduate)
  • Airman License (Issued August 2016 onwards)
  • Valid ePassport (for renewal)
  • Residence Card (for applicants overseas)


Note: Acceptable IDs must have entries (i.e. Full Name, Birthday and Gender) that is consistent with the entries on the birth certificate. ANY FORM OF DISCREPANCY WILL NOT BE RECOGNIZED THROUGH AN AFFIDAVIT.

REMINDERS


  1. Personal appearance of every applicant is a MUST.
  2. Personal appearance of either parent for minor applicants is a MUST (preferably mothers).
  3. Applicants must bring their printed Application Packet
  4. Payment is NON-REFUNDABLE.
  5. Each applicant should bring an exact amount of 150 php for courier.
  6. POW is one day processing. There will be no extension of appointment and each transaction non-transferrable. No Show is forfeiture of payment and no reschedule.
  7. Requirements must be complete on the day of processing.
  8. Please be reminded that if an application is not approved due to discrepancy or missing document, payments are forfeited and there will be no extension of appointment and processing.
  9. Applicants MUST bring the original and photocopies of their documents
  10. Applicants with lost expired and lost valid (Passports issued 2010 and above) are not eligible for Passport on Wheels.


FAQs


  • Applicants with blurred/unreadable PSA birth certificates and marriage certificates are advised to get Local Copy from their Local Civil Registrar Applicants with blurred/unreadable PSA BCs and PSA MCs
  • Minor Applicants not accompanied by Mother (and is instead accompanied by the Father) must bring Original PSA Marriage Certificate.
  • If either parent is overseas, Special Power of Attorney and Affidavit of Support and Consent (SPA and ASC must be authenticated in Philippine Embassy where the parent resides) and DSWD Clearance must be submitted.
  • Applicants with discrepant documents (BCs, MCs, and IDs) are advise not to apply.
  • Late Registered PSA Birth Certificates must submit recent NBI Clearance and an Acceptable Valid ID that pre-dates the Late Registration (preferably Voter’s Certification).
  • Infant applicants (Only those within aged five (5) months upon their birth) without PSA Birth Certificates should instead bring Certified True Copy (CTC) of birth certificate from Local Civil Registrar and authenticated by PSA.
  • For those who have no record of PSA Birth Certificate:
  • Born in 1950 and above, must first be registered at their Local Civil Registrar and have it forwarded at PSA. They must be registered and must be able to present an original PSA Birth Certificate and NBI Clearance.
  • Born in 1949 and below, present a Joint Affidavit of Two Disinterested Persons, Negative Birth Record from PSA and NBI Clearance.
